APPLICATIONS

All applications are PDF documents that can be viewed and printed with Adobe Reader. If you do not have
Adobe Reader, it can be downloaded (FREE) at the Adobe website (www.adobe.com).

I. Single Dog Registration:
In order to complete the registration process, you must complete the following steps:

1.     Complete pages 1 & 2 of the Single Dog Registration Application.
       **Note: Dog must be at least 6 months old.**

2.     Provide 2 color photographs of the dog: 1) a front head and chest view and 2) a side head, body, and tail view.
        **Note: Make sure photos are clear.**
       ** Photos are not required if dog is registered with the A.D.B.A., A.R.B.A., B.F.K.C., or U.K.C.**

3.     Include a single registry fee of $15.00 per dog, which includes 4-generation pedigree, in the form of a money order
        made payable to: American Pit Registry.

4.     Please mail application, photographs, a 3-generation pedigree, and fee to: A.P.R., 247 Doone Road, Fairless Hills,
        PA 19030. If you have any questions, feel free to contact the A.P.R. at americanpitreg@americanpitregistry.com.

II. Litter Registration when sire and dam are register with A.P.R:

1.     Complete pages 1 & 2 of the Litter Registration Application.

2.     Include a litter registry fee of $10.00 for litters of 8 puppies or under and $1.00 for each additional puppy, in the
        form of a money order made payable to: American Pit Registry.

3.     Please mail application and fee to: A.P.R., 247 Doone Road, Fairless Hills, PA 19030. If you have any questions,
        feel free to contact A.P.R. at americanpitreg@americanpitregistry.com.

**Sire and dam must be in current owner's name. If a transfer of ownership is required, the form must be submitted along
        with Litter Registration Application.

III. Litter Registration when sire, dam, or both are not registered with A.P.R:
In order to register a litter, both sire and dam must be registered with A.P.R.

1.     Complete the Single Dog Registration process for each dog that is not registered - instructions are provided in
        Section I.

2.     Complete pages 1 & 2 of the Litter Registration Application, which can be downloaded below.

3.     Include a litter registry fee of $10.00 for litters of 8 puppies or under and $1.00 for each additional puppy, in the
        form of a money order made payable to: American Pit Registry.

3.     Please mail all application materials and fees to: A.P.R., 247 Doone Road, Fairless Hills, PA 19030. If you have
        any questions, feel free to contact A.P.R. at americanpitreg@americanpitregistry.com.

IV. Application for Pedigree:

1.     Complete the Application for a 3-Generation Pedigree or 4-Generation Pedigree.

2.     Include a fee of $12.00 for each 3-Generation pedigree and $15.00 for each 4-Generation pedigree requested in
        the form of a money order made payable to: American Pit Registry.

3.     Please mail application and fee to: A.P.R., 247 Doone Road, Fairless Hills, PA 19030. If you have any questions,
        feel free to contact A.P.R. at americanpitreg@americanpitregistry.com.

V. Application for Duplicate Certificate or Pedigree:

1.     Complete the Application for Duplicate Registration Certificate and/or 3-Generation Pedigree.

2.     Include a duplicate fee of $10.00 for each registration certificate and $15.00 for each pedigree requested in the
        form of a money order made payable to: American Pit Registry.

3.     Please mail application and fee to: A.P.R., 247 Doone Road, Fairless Hills, PA 19030. If you have any questions,
        feel free to contact A.P.R. at americanpitreg@americanpitregistry.com.

VI. Change of Address Application:

1.     Complete the Change of Address Application. No fee required to change the address in our records.

2.     Include a fee of $8.00 for a new certificate and 3-generation pedigree or $10.00 for a new certificate and
        4-generation pedigree.

3.     Please mail application and fee to: A.P.R., 247 Doone Road, Fairless Hills, PA 19030. If you have any questions,

VII. Re-Type Information Application:

1.     Complete the Re-Type Information Application. No fee required to change information in our records.

2.     Include a fee of $5.00 for each new document.
